Raise a concern

Use this form to raise a fitness-to-practise concern about a person registered with HPAC. All fields marked required must be completed.

About you and the concern
Please be aware that unless there are mitigating circumstances, like most regulators we are not usually able to take forward a concern that is made anonymously, or if the person making the complaint (complainant) wants to stay anonymous. This is because we want to operate a fair and transparent process. The name of the person making the referral must be provided, as well as their date of birth, do not use your organisation's name. There is a separate section for that. Failure to comply will result in your complaint not being progressed. Additionally, we can not accept anecdotal or hearsay reports. Evidence will be required to verify your concern. If a concern relates to sexual safeguarding, a signed statement from the alleged victim or the alleged victim's carer will meet the threshold to trigger an investigation. If you are a current or previous employer you will have to show that you conducted your own investigation and share your investigation process and evidence of your findings with us. If you believe the individual you are reporting has committed a crime, in addition to raising this concern you should inform the police directly.
